Reporting Type 1 Occurrences
What is a Type 1 occurrence?
Type 1 occurrences are incidents that may or did result in serious harm to a patient in relation to Intravenous Infusion Therapy (IVIT) procedures. It’s critical that the College be made aware of these events so that we can take appropriate action to protect the public from unsafe care.
Part IV of the General Regulation outlines the following events as Type 1 Occurrences:
- The death of a patient at the premises after a procedure was performed.
- The death of a patient that occurs within the five days following the performance of a procedure at the premises.
- Any referral of a patient to emergency services within the five days following the performance of a procedure at the premises.
- Any procedure performed on the wrong patient at the premises.
- The administration of an emergency drug to a patient immediately after a procedure was performed at the premises.
- The diagnosis of a patient with shock or convulsions occurring within the five days following the performance of a procedure at the premises.
- The diagnosis of a patient as being infected with a disease or any disease-causing agent after a procedure was performed at the premises, if the Registrant believes that the patient may have been infected due to the performance of the procedure.
Reporting requirements
All Type 1 occurrences must be reported to the College through the Type 1 Occurrence Report Form, within 24 hours of learning of the event.
Be aware that this requirement applies to all Registrants, not just those who performed the IVIT procedure. That means that all Registrants who learn of a Type 1 occurrence must also report it to the College within 24 hours of learning of the event. This ensures that the report does not slip through the cracks.
